| 1) Increased risk with DES vs. BMS in Saphenous Vein Graphs (SVG)? Dr. Sanjay Kaul, FACC and director of the Vascular Physiology and Thrombosis Research Laboratory at Cedars-Sinai Medical Center (Los Angeles), and colleagues discussed the use of drug-eluting stents (DES) vs. bare metal stents (BMS) in the July 10, 2007 issue of the Journal of the American College of Cardiology. The authors noted a Dutch study which included 75 people who had previously received SVGs in 96 vessels. During the study, 38 patients received 60 sirolimus-eluting stents in 47 vessels, while 37 patients received 54 BMS in 49 vessels. Of the 38 patients who received a DES, 11 (29%) died over a 32-month follow-up period while no deaths were observed in the BMS group. Dr. Kaul noted that the results of the Dutch study are not surprising because the "FDA approved stents on the basis of a highly select patient population, which did not have the characteristics of the people in this particular trial." According to Dr. Kaul, it is not currently possible to definitively state why a DES might present a great hazard in a SVG. However, he noted that SVGs often have softer plaques and can accelerate the process of atherosclerosis. Dr. Kaul argued that that more studies of the safety of DES in SVG are needed. "Until a definitive study comes out which demonstrates that they are safe, I would stick with balloon angioplasty and bare-metal stents. They are not very effective, but they are safe," he said. | 2) Spectranetics TURBO-Booster FDA cleared to treat arterial occlusions in the leg Spectranetics announced July 2007 that it has received FDA clearance to market its TURBO-Booster product for the treatment of arterial stenoses and occlusions in the leg. This represents a broader indication for use compared to existing peripheral laser catheters. The TURBO-Booster functions as a guiding catheter facilitating directed ablation of blockages in the main arteries at or above the knee. The TURBO-Booster combined with Turbo elite laser catheters allows for removal of large amounts of plaque material within the SFA and popliteal arteries. The CELLO trial provided data to the FDA in support of the 510(k) for the TURBO-Booster. The study was a prospective registry with 61 patients in 16 centers within the US. Patients were included with stenoses and occlusions ≥ 70% and ≤ 100% of the vessel lumen in arteries from 4 to 7 mm in diameter. The reduction in % diameter stenosis was 35% following use of the TURBO-Booster with no major adverse events through 30 days.
